MACRA. MIPS. New law. Proposed rule. “The end” of Meaningful Use. New payment models. Your head spinning yet? If so, you’re not alone. Let me help you catch your breath and hopefully clear up some confusion.

If you were skeptical that CMS intended to move reimbursement toward value-based care, the passing of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), should put your doubts to rest. This bill was approved by the House and Senate with the overwhelming bipartisan support of more than 90 percent of lawmakers (unlike HITECH and ACA), so based on this alone (not to mention other reasons to complicated to get into here) it’s not going away.

What is MACRA?

Consistent with earlier policy (see MU, PQRS, CMMI and a bevy of other acronyms) and the widely-held belief that moving away from fee-for-service is the only way to reign in out-of-control healthcare costs, Congress and the President provided certainty last year in the form of a new law. This law (and the proposed rules that followed) introduces a new value-based payment system consisting of two pathways:  the Merit-Based Incentive Payment System (MIPS) and Advanced Alternative Payment Model (APM).

On April 27, 2016, the Centers for Medicare and Medicaid Services (CMS) issued proposed regulations implementing key provisions of MACRA with a proposed start date of January 1, 2017; a final rule is expected by November, 2016. We will keep you posted on developments with the final ruling.
